Booking Form isn't Displaying

    I’m having an issue with the booking form on my Pro license.

    The booking form was showing up earlier today, and now it’s disappeared.

    Here is the link to view the page where it should show up:

    Can you help me figure out what is wrong?

    Hi Jason,

    I’ve just sent you an email regarding this. There are number of reasons why this could happen, so it will probably be easier to determine the cause directly rather than through the forums.
